MAT Registration

PRE-REGISTRATION IS REQUIRED FOR THE MAT. Pre-registration requires return of this form and remittance of $70.00. In order to take the examination, you must present two (2) forms of identification (one with a photograph) at the examination site. If you have taken the MAT during the last 12 months you must present a retest admissions ticket at the examination site in order to retake the MAT.
